Medical report request

Use this service to request a medical report.

Please note that making a request for a report does not guarantee one will be completed, as your request may need to be discussed with the doctor.

Some reports require a fee to be paid in advance and/or physical examination to take place.

The GP is unable to write a report for PIP/DLA or similar benefit claims using this service; if the benefit assessment service require a report they will contact the GP directly to request this.
